I understand that MTS will retain this application whether or not it is approved. MTS and/or its assigns are authorized to check the credit and employment history of the Applicant and the undersigned, obtain insurance information and may also disclose your personal information to third parties and credit reporting agencies. The applicant authorizes MTS and its assigns to contact Applicant’s creditors and authorizes any creditor so contacted to release to MTS and its agents or assigns such credit information as may be requested.
